Rental Application Form
Once you have viewed a rental and wish to apply you can print off our application form below and then scan and email it to email@example.com or fax to 5222 5543. Please ensure you complete as much information as possible, that you sign the Privacy Statement and send through your 100 points of ID. The application process takes approximately 48 hours. Please note that only successful applicants are notified. If you wish to check on your application you are welcome to contact our office on 5228 3333 during business hours.

Notice to Vacate Form
Please forward the completed Notice to Vacate form to our office in person or by mail at 187a Moorabool Street, Geelong, 3220 or by fax to 5222 5543 or email to firstname.lastname@example.org. This notice must be received no less than 28 days prior to your intended vacating date at the end of your lease. The 28 days will commence from receipt of the completed form in our office. Please note you must receive confirmation of receipt of your Notice to Vacate from the Property Manager by return email or phone call. Please contact our office within 24 hours if you have not received a confirmation from us.
Should you wish to vacate earlier than the end of your lease please contact your Property Manager for lease break conditions and the appropriate forms.

If you have a maintenance issue please contact our office during business hours on 5228 3333 or print off and complete the maintenance request form below and return it to our office in person or on fax no. 5222 5543. If the maintenance issue is of an urgent nature and out of hours please phone 0409 192 290.
Urgent maintenance issues are defined as follows:
· ANY BURST WATER SERVICE
· ANY SEWERAGE BLOCKAGE
· ANY BROKEN SEWERAGE FITTINGS
· ANY SERIOUS ROOF LEAK
· ANY GAS LEAK
· ANY ELECTRICAL FAULT LIKELY TO CAUSE DAMAGE TO PROPERTY OR ENDANGER HUMAN LIFE
· ANY FAULT IN A LIFT IN A RENTED PREMISES
· ANY SUBSTANTIAL DAMAGE CAUSED BY FLOODING, STORM OR FIRE
· ANY DAMAGE OF A PRESCRIBED CLASS
Please note that consumables such as light globes, remote control batteries and smoke detector batteries need to be replaced as required by the tenant. If a fault continues, after the battery change, contact us during office hours on 5228 3333.
If your maintenance issue is in regard to a pilot light for a gas appliance you must try and relight in the first instance. If after attempting this it is still not working contact our office.
In order to quickly attend to your maintenance issue we need to have as much detailed information as possible about the problem. You can assist us by providing the make and model of the appliances, giving us a clear description of the problems and making sure that we have your best contact numbers.
